Position: Community Based Clinician
Department: Living Well at Home
Location: Cambridge, MA
Schedule: 40 hours per week, On-site
POSITION SUMMARY:
The Living Well at Home Community Based Clinician provides on-site therapy services to individuals living in congregate living settings in Cambridge, MA. The Community Based Clinician will administer relevant assessments including PHQ-9, develop treatment plans, and provide culturally appropriate and trauma informed therapy services in the building and, on occasion, in the community.
JOB RESPONSIBILITIES:
Manage clinical needs of complex case load of clients with serious mental illness and/or substance use disorders
Meet with clients in various community settings, not limited to, but including, the home and shelter environments
Support client driven therapeutic goals by incorporating the appropriate interventions into therapy sessions
Complete clinical documentation, assessments, and service plans in a timely manner
Work in collaboration with the geriatric psychiatrist and clinical housing director on program development, execution, and evaluation
Actively participate in all scheduled meetings, including team meetings, group supervision, and individual supervision
Using a trauma and harm-reduction informed lens, maintain appropriate boundaries with clients and staff
Utilize excellent time management skills

JOB REQUIREMENTS
REQUIRED EDUCATION AND EXPERIENCE:
Master’s degree in social work or mental health counseling required.
PREFERRED EXPERIENCE:
3+ years working with patient populations impacted by serious mental illness and substance use disorder strongly preferred.
Experience working in a congregate living setting and/or homeless services preferred
Experience working in an independent role preferred
CERTIFICATIONS, LICENSES, REGISTRATIONS REQUIRED:
Requires a Massachusetts license as an independently licensed mental health professional such as Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC) within 6 months of hire.
KNOWLEDGE, SKILLS & ABILITIES:
Ability to work with a diverse and highly impacted client population using a person-centered approach
Comprehensive knowledge of behavioral health and housing resources and systems, specifically in Greater Boston Area
Skilled at engaging difficult to engage patients—build rapport, trust and utilizing de-escalation skills
Ability to work in a fast paced environment, inclusive of organizational skills, time management skills, critical thinking, and problem solving abilities
Strong interpersonal skills and ability to communicate in a courteous, pleasant and professional manner with healthcare providers, patients and families, general public, staff members, external agencies, and other organizational leaders.
Ability to work independently and to make decisions based on established health system policies and procedures as well as the NASW or professional organization code of ethics
Proficient oral and written English communication skills. Bi-lingual and/or bi-cultural candidates preferred
Intermediate Microsoft Office suite experience (i.e. MS Word, Excel, Access, Outlook)
Experience using an electronic health record preferred

Compensation Range:
$78,000.00- $113,000.00
This range offers an estimate based on the minimum job qualifications. However, our approach to determining base pay is comprehensive, and a broad range of factors is considered when making an offer. This includes education, experience, skills, and certifications/licensures as they directly relate to position requirements; as well as business/organizational needs, internal equity, and market-competitiveness.
